Deadlines & Registration Rates

Before registering, your organization should decide which person is registering as the first attendee and which are registering as additional attendees — additional attendees are charged at a lesser rate. Each person registers individually using this form and selects their own rate.

Payment

Payment is collected after you submit this form. A link to your invoice will be provided at the conclusion of the form, and an invoice will also be emailed to you. You can pay online via ACH or credit card. Please complete payment promptly to confirm your registration.

If you prefer to pay by check, make your check payable to NMDA and mail to: 5701 N. High St., Ste 201, Worthington, OH 43085.

Hotel Accommodations

The event is held at the Embassy Suites by Hilton Chicago O’Hare/Rosemont. Hotel accommodations are separate from your registration fee and are booked directly with the hotel.

This is the form to register your attendance and identify yourself as a meeting room coordinator for your organization. It is not your hotel booking reservation. After your registration is received, you will be sent hotel booking instructions via email which are specific to your registration responses. Please wait for those instructions before booking your room — do not book directly through the hotel website, as NMDA has reserved room blocks with special instructions for you.

Hotel booking instructions are sent on a weekly basis. If you have not received yours within one week of registering, please contact us at the address below.
